For this form only a few of the fields are actually required(*) to be filled out. So, if you’re just offering a tip of a zero waste business you love or know about, it’s a really simple application and you only need to fill in 9 fields, but you are more than welcome to fill in all applicable fields too.

However, if you are the business owner, associate, or representative your application will go much faster if you add as much information about your business as you can.
